Microscopically Assisted Dentistry

In my mind the greatest advancement in dentistry has been the use of magnification.   When I was in dental school dentists were just beginning to use loupes for magnification.   Over the next 20 years they progressed to small telescopes on the dentists glasses.   These provided good magnification, but were very hard on the operator because of the weight, small field size and depth and the method of focusing them.   

In 1999 I was introduced to the operating microscope. Microscope I immediately saw the advantages.   For me, it was more comfortable to use, and the size and depth of field were much larger.   Also since the light source was incorporated I had better lighting on the work area. I was able to go from 2.5X magnification with loupes to 6X, 10X even up to 24X with the microscope.   To put this in perspective, the unaided human eye can see a resolution of about 200u (microns).   Using 2.5X loupes, we get to about 100u, a sharp explorer can detect about 40u, the microscope at 8X 25u, at 10X 20u, at 14X 14u, and at 20X 10u.   So at 20X magnification I can see things that are 1/10th the size they would have to be to be seen by traditional dental loupes.    

All of our treatment rooms have microscopes with live video feed to a monitor on the ceiling so the patient can see as much of the procedure as they want to.  This is especially useful during exams and consultations.  There is also a feed to a monitor behind the patient for my assistant to see what I am doing, making her much more efficient. 

For the patient this all translate into a more conservative, less traumatic approach to dentistry.
