Dental Cosmetic Concerns: White Spots
  

 

One of dentistry’s more frustrating cosmetic concerns is white spots on the teeth. White spots show up for a variety of reasons, the most common are from too much fluoride while the teeth are growing, and poor oral hygiene while wearing braces.  New advances in dental research provide some resolution for these spots.

 

The white spots are where there is a different amount of calcification in the enamel compared to the surrounding tooth structure.  Our goal is to even the density and therefore the color for the entire tooth. While we cannot get everything exactly the same color, we can vastly improve the esthetics.

 

To accomplish this we use both an in office and at home treatment protocol, and usually combine this with bleaching.  Meticulous oral hygiene in required to see improvement.

 

In the office, we first isolate the teeth to be treated. We then clean them with plain pumice and etch them with a mild acid etch.  This step is repeated 3 to 4 times and evaluated for the frosty appearance that indicates the enamel tubules are open and ready to receive the recalcifying paste.  The paste is the applied to the treatment area and allowed to work for 5 minutes.  This in office treatment is repeated every 3 weeks for 6 months.

 

While undergoing the in office portion of the treatment, the patient is given trays to wear at home twice per day for the duration of the treatment.  The same recalcifying paste we use in the office is used in the trays at home.  The patient in instructed to keep the teeth clean and avoid staining foods such as dark colored soda pop, grape juice, coffee, tea, red wine and smoking.
